Thelargestemploymentsettinginhealthcareishospitalsandthelargestcategoryofhealthcareworkersisregisterednurses,with2.7millionjobs,61%ofwhichareinhospitals(BLS,2014h).AccordingtotheBLS,therewere691,400physiciansandsurgeonswhoheldjobsin2012(BLS,2014e).Increasingly,physiciansarechoosingtopracticeinlargegroupsortobeemployedbyhospitals,ratherthaninsoloorsmallpractices.In2013,JacksonHealthcarere-conductedasurveyofphysiciansandfound26%wereemployedbyhospitals,anincreaseof6%overthepreviousyear.Ownershipstakesinpractices,solopractices,andindependentcontractorstatusesalldeclinedinthesameperiod(Vaidya,2013).Employmentoffersphysiciansasafehaveninavolatilehealthcareenvironment.Undertheumbrellaofahospitalorotherlargehealthcareorganization,theyhavebetterworkhours,benefits,andtimeoff,whichtheycouldnotalwaysaffordinsmallorsolopractice.Itisexpectedtheproportionofemployedphysicianswillcontinuetogrowinthecomingdecade.In2012,physicianassistantsheld86,700jobs,over55%ofwhichwereinambulatoryhealthcareservices,includingphysicianpractices,about20%wereinhospitals,andtherestinnursingcarefacilitiesandgovernmentsettings(BLS,2014f).Alliedhealthprofessionalsconstituteabroadarrayof28healthscienceprofessions,including,butnotlimitedto,anesthesiologistassistants,medicalassistants,respiratorytherapists,andsurgicaltechnologists(CommissiononAccreditationofAlliedHealthEducationPrograms,2015). Thesestatisticsmeanthat,asahealthcaremanager,inmanyinstancesyouwillbeworkingwithamixofpeoplewitheithermoreorlesseducationthanyouhave.Italsomeansyouwillnothavetheclinicalcompetenciesthatthesehealthcareprovidershave—anintimidatingscenario,tosaytheleast.Insteadofclinicalexpertise,however,youwillbringabackgroundthatenablesyoutoenhancetheenvironmentinwhichthesehighlyspecializedpersonneldeliverhealthcareservices.Youwillbethepersonresponsibleformakingsurenurses,doctors,andotherhealthcareprofessionalshavetheresourcestoprovidesafeandeffectivepatientcare.Yourrolewillbetoprovideandmonitortheinfrastructureandprocessestomakethehealthcareorganizationresponsivetotheneedsofthepatientsandtheemployees.Themoreyouunderstandclinicalhealthcareprofessionals,thebetterpreparedyouwillbetodoyourjobasahealthcaremanager.Thepurposeofthischapteristoprovideyouwithanoverviewofwhoyourfuturecolleaguesare,howtheyweretrained,andwaystomanagethequalityoftheirworkenvironment.PHYSICIANSPhysiciansbegintheirpreparationformedicalschoolasundergraduatesinpremedicalprograms.Premedicalstudentscanobtainadegreeinanysubject;however,theAssociationofAmericanMedicalColleges(AAMC)(2015)indicatesthattheexpectationisthattheywillgraduatewithastrongfoundationinmathematics,biology,chemistry,andphysics.Entryintomedicalschooliscompetitive;applicantsmusthavehighgradepointaveragesandhighscoresontheMedicalCollegeAdmissionTest(MCAT).
