Biliary obstruction due to peripapillary duodenal diverticulum is a rarely mentioned cause that sometimes makes it difficult for doctors to find the cause of biliary obstruction when performing Endoscopic Retrograde Cholangio Pancreatography (ERCP). We report 3 cases of obstructive jaundice due to peripapillary diverticulum who underwent ERCP. Objective: Describe the historical, clinical, paraclinical characteristics and survey the diagnostic issues of cases of biliary obstruction due to peripapillary diverticulum at Thong Nhat Hospital, Ho Chi Minh City Patients and method: Retrospective report of 3 cases of biliary obstruction due to peripapillary diverticulum diagnosed and treated at Thong Nhat Hospital, Ho Chi Minh City from February 2022 to June 2023 Results: From February 2022 to June 2023, we had 3 cases of biliary obstruction diagnosed due to peripapillary diverticulum. All 3 patients were over 60 years old, with clinical symptoms of obstructive jaundice, biliary colic-like abdominal pain, and biliary infection; Tests showed increased Billirubin and increased liver enzymes. Imaging including ultrasound, computed tomography (CT scan), and magnetic resonance cholangiopancreatography (MRCP) showed dilatation of the intrahepatic and extrahepatic bile ducts but no cause of obstruction such as stones or tumors was found. ERCP results: presence of peripapillary diverticula, no cause of obstruction such as stones or tumors was found. All 3 patients received Oddi myectomy. Conclusions: Peripapillary diverticulum is a cause of biliary obstruction that is rarely mentioned in clinical practice. Imaging such as CT scan and MRCP help guide diagnosis. ERCP helps confirm the diagnosis and treatment with Endoscopic Sphincterotomy.
