Nội soi một trocar hỗ trợ cắt u nang buồng trứng lành tính ở trẻ em

Nghiên cứu với mục tiêu nhằm đánh giá tính khả thi, an toàn và hiệu quả của kỹ thuật nội soi một trocar hỗ trợ trong phẫu thuật cắt u nang buồng trứng lành tính ở trẻ em. Nghiên cứu thực hiện ở các bệnh nhi với chẩn đoán u nang buồng trứng lành tính được phẫu thuật nội soi một trocar từ tháng 2/2009 đến tháng 5/2013.

For thesingle trocar laparoscopic‐ assisted surgery, a 10 mm umbilical trocar was placed and a 10 mm camera withengrafted5mmworkingchannelwasused.Theovariancysticwallwasgraspedandbroughttotheumbilicalsiteand then the cystic fluid was aspirated outside the peritoneal cavity. Incaseofshortadnexalpedicle,makingdeliveryofthecysttotheumbilicalsiteimpossible,aspirationofthecystwasperformedbyneedlepuncturethroughtheabdominalwallunderthelaparoscopiccontrol.Thecystthenwasdeliveredoutoftheabdomenviaaminimaltransversalsuprapubicincisionandcysticremovalwasperformedasdescribed.Results: 23 patients were identified, with median age of 3 years (ranged from 8 days to 10 years). Themediansizeofthecystwas7.0cm(rangedfrom3cmto11cm).In12casesthecystwasmatureteratoma,in7–simplecystandin4–dermoidcyst.In19patientsthecystwasexcisedviatheumbilicalincisionandin4‐viathesuprapubicincision.Ovariantissuesparingwasachievedin20cases(87.0%).Themeanoperativetimewas46.5±15.1minutes(ranged30‐90minutes).Therewasnoconversion,nointra‐orpostoperativecomplication.Themeanpostoperativehospitalstaywas1.8±0.6days(ranged1‐3days).Forthefollowupfrom1‐50months,allpatientswereingoodhealth,withoutrecurrence.Thepostoperativecosmesiswasexcellent.Conclusions: Single trocar laparoscopic‐ assisted surgery is highly feasible, safe, effective, with excellentcosmesisinmanagementofbenignovariancystic ...
