Kết quả bước đầu về nghiên cứu giải phẫu ứng dụng vách sau khớp gối

Mục tiêu nghiên cứu là xác định đặc điểm giải phẫu ứng dụng VSKG: vị trí, giới hạn, liên quan VSKG với các cấu trúc lân cận từ đó ứng dụng kĩ thuật thực nghiệm thành lập điểm xuyên vách.

Keát quaû: Vaùch sau khôùp goái coù hình tam giaùc, giôùi haïn roõ phía tröôùc laø daây chaèng cheùo sau, phía sau laø bao khôùp goái sau, vaø phía treân laø bôø sau hoá gian loài caàu. Vaùch sau naèm leäch veà phía khoang sau trong cuûa khôùp goái. Ñoäng maïch khoeo naèm phía sau ngoaøi vaùch sau vôùi khoaûng caùch trung bình khoaûng 6mm. Khi xuyeân kim Kirschner theo höôùng töø cöûa sau ngoaøi qua cöûa sau trong khôùp goái luoân ñi qua vaùch sau vaø khoâng gaây toån thöông caùc caáu truùc khoang sau goái. Keát luaän: Ñöôøng thaúng noái cöûa sau ngoaøi vaø cöûa sau trong luoân ñi qua moät ñieåm treân vaùch sau vaø khoâng gaây toån thöông caùc caáu truùc khoang sau khôùp goái. Do ñoù, ñöôøng thaúng naøy coù theå öùng duïng taïo ñieåm xuyeân vaùch an toaøn vaø hieäu quaû Clinical anatomy of the posterior septum of the knee A preliminary result of study Le Viet Can*, Summary Do Phuoc Hung*, Trang Manh Khoi** Background: The anatomical study of the posterior septum of the knee helps to establish safely and effectively the transseptal portal in arthroscopic surgery of the posterior knee compartments. Purpose: determine characteristics of clinical anatomy of the posterior septum of the knee. Material and method: We conducted a cross-sectional descriptive study in 22 cadaveric knees at Anatomy Department of University of Medicine and Pharmaceutical of HCMC and 14 fresh knees that are amputated at Cho Ray Hospital. Result: The posterior septum is triangular in shape, bounded by the posterior cruciate ligament anteriorly, the posterior capsule posteriorly, and the posterior portion of the femoral intercondylar notch superiorly. The septum devides the posterior compartment of the knee joint into posteriomedial and posteriolateral comparments, and is located medial deviation of posterior compartment. The popliteal artery is consistenly located poterolateral to the septum and is distant a mean of 6mm. When the Kirschner wire is penetrated from the posteriorlateral portal to the posteriomedial portal of the knee, it always goes through a safe point on the posterior septum . Conclusion: A straight line linking posterolateral portal and posteromedial portal of the knee consistenly goes through one point on the posterior septum and doesn’t damage any structure in posterior compartment. Threrefore, following this line will be safe to establish a transseptal portal. Key words: posterior septum, transseptal portal, knee arthroscopy. 338Đặt vấn đề: chung.
