Yếu tố nguy cơ, yếu tố bảo vệ đối với quan hệ tình dục ở vị thành niên

Nghiên cứu này sử dụng phương pháp nghiên cứu định tính kết hợp định lượng để tìm hiểu yếu tố nguy cơ, yếu tố bảo vệ đối với hành vi quan hệ tình dục ở vị thành niên. Thông tin định tính được thu thập bằng kỹ thuật nhóm đề cử/bầu chọn (Nominal group technique). Số liệu định lượng được trích xuất từ điều tra dọc về sức khỏe thanh thiếu niên (năm 2006, 2007 và 2009) tại Chililab, Chí Linh, Hải Dương- cơ sở thực địa đào tạo, nghiên cứu của trường Đại học Y tế Công cộng.

Soá lieäu ñònh löôïng ñöôïc tríchxuaát töø ñieàu tra doïc veà söùc khoûe thanh thieáu nieân (naêm 2006, 2007 vaø 2009) taïi Chililab, Chí Linh,Haûi Döông - Cô sôû thöïc ñòa ñaøo taïo, nghieân cöùu cuûa Tröôøng Ñaïi hoïc Y teá Coâng coäng.Tyû leä (hieäu chænh)VTN ñaõ QHTD laø 1,7% nam vaø 0,4% nöõ (ñieàu tra naêm 2006), 4,9% nam vaø 1,9%nöõ (ñieàu tra naêm 2009). Tyû leä môùi QHTD (incidence) laø 44/1000 nam/3 naêm vaø 19/1000 nöõ/3 naêmgiöõa 2 voøng ñieàu tra (2006 vaø 2009). Tuoåi trung bình (hieäu chænh) QHTD laàn ñaàu laø 16,2 tuoåi (nam),17,2 tuoåi (nöõ). Tyû leä QHTD laàn ñaàu töï nguyeän laø 81% (nam) vaø 45% (nöõ). Caùc yeáu toá nguy cô, yeáutoá baûo veä ñoái vôùi haønh vi QHTD ôû nam, nöõ vò thaønh nieân ñöôïc xaùc ñònh. Nhöõng yeáu toá naøy laø caên cöùcho caùc chöông trình truyeàn thoâng, giaùo duïc, can thieäp coäng ñoàng giaûm nguy cô QHTD, quan heä tìnhduïc khoâng an toaøn vaø naâng cao söùc khoûe sinh saûn ôû VTN.Töø khoaù: vò thaønh nieân, yeáu toá nguy cô, yeáu toá baûo veä, quan heä tình duïc, nghieân cöùu doïc.Risk and protective factors for sexualintercourse among adolescentsNguyen Van Nghi(*), Vu Manh Loi(**),Le Cu Linh(***), Nguyen Huu Minh(****)This research employed both qualitative and quantitative methods to identify risk and protectivefactors relating to sexual behaviors among adolescents. The qualitative data was collected by usingNominal group technique. Quantitative data was extracted from the adolescent health researchproject conducted in Chi Linh district, Hai Duong province - a research and training field site ofHanoi School of Public Health.Quantitative results show that adjusted percentage of adolescents having sexual intercourseexperiences is 1.7% for boys and 0.4% for girls ( 2006 survey), 4.9% for boys and 1.9% for girls (2009survey). The incidence of having sexual intercourse is 44/1000 boys per 3 years and 19/1000 girlsper 3 years (2006 to 2009). Adjusted average age at the first sexual intercourse is 16.2 years old forboys and 17.2 years old for girls. The risk factors relating to sexual intercourse among boys are foundin the age group of 15-19 years; urban residents; low secure community; family domestic violence;Taïp chí Y teá Coâng coäng, 7.2010, Soá 15 (15)33| TOÅNG QUAN & NGHIEÂN CÖÙU |having deviant friends (smoking, drinking); frequent internet use and game playing . The protectivefactor relating to sexual intercourse among boys consists of having mothers taking care among theage group of 10 - 14 years. The risk factors relating to sexual intercourse among girls are physicaland mental abuse in family (being beaten and scold at in the family); having deviant friends (smoking,drinking); internet use and game playing. The protective factors relating to sexual intercourseamong girls are teachers encouragement and fair treatment in school. The qualitative results showthat risk factors relating to sexual intercourse among adolescents (boys and girls) are watching pornmovies; being curious and interested in sexual matters; being sexual abused; weak familymanagement and support; unhappy family (divorced, conflict); low communication with parents;dropping out of school or no schooling; being bullying; bad behaviors (school record); early love;access to sex workers; availability of porn CDs, DVDs (Italics is the most important one). Protectivefactors relating to sexual intercourse among adolescents are reasonable perception on love andmarriage; having well-behaved friends; good family management and support; good communicationwith parent; happy family; good behavior (school record); fair treatment in school; no porn moviewatching; no service of porn CD/DVD and no sex workers in the community.Key words: Adolescents, risk and protective factor, sexual intercourse, longitudinal study.Taùc giaû(*)Ths.
